You see, the problems will persist. There is no, absolutely no accountability at the Registry or any government departments in St Lucia.
The clerk who types information in the system should have had someone, a supervisor or superior checking his/her work. If errors found, that clerk should be re-trained or reprimanded. If they knew that there would be repercussions for errors, I can guarantee this silliness would stop.
"Everyone is expected to make mistakes, especially on the job. One of the supervisor’s primary responsibilities is to correct mistakes made by his or her staff. When a supervisor does not point out a mistake and does not guide the staff person to the correct way of proceeding, the supervisor neglects both the staff person and the goals of the organization."
